Scheduling

When scheduling your first appointment, be sure to complete the Northside Outpatient Intake Forms before attending your first session. For rescheduling, please provide at least 48-hours notice if you need to schedule or cancel an appointment.

Billing

All co-payments are due at the time of service. You can pay by phone, by mail, or in person at your next appointment. We accept MasterCard, Visa, Discover Card, Cash, or Check.

Northside will not deny access to services due to inability to pay; and there is a discounted/sliding fee schedule available.
