Community Support Services

Providing a positive alternative to long-term hospitalization with residential, social, employment rehabilitation, and case management services. 

Case Management

Staff members provide professional assessment, planning, and monitoring of each client’s needs and provide continued advocacy. Clients are referred to appropriate community resources and services while continually monitoring and assessing needs. Clients are generally seen one to two times per month.

All Community Support Services are referral based.

Residential Services

We connect clients with housing based on the client's needs including, group homes that are most restrictive, supervised apartments that are less restrictive, and satellite apartments for independent living. HUD subsidizes rent payments based on income.

Group Homes: 12-15 beds available. Here, clients learn basic skills and learn how to trust themselves and build upon individual strengths. Positive behaviors are learned and reinforced. Group homes are restrictive and staffed 24 hours per day.

Supervised Apartments: 40 beds available. Individuals assume greater responsibility for planning and maintaining living skills while becoming independent with others. Supervised apartments are less restrictive and staffed 24 hours per day.

Satellite & Community Apartments: 14 beds available. These facilities are where individuals live independently with minimal support staff, emphasizing peer support and individuals helping each other solve day-to-day problems.

Residential Services are referral based.

Medication Case Management

Our Medication Case Managers meet with clients at least every six months for treatment planning and referrals to available community resources. In addition, your matched psychiatrist or nurse practitioner will work with you on medication management. This service is a good fit for clients taking medication and who cannot commit to therapy sessions.

Florida Assertive
Community Treatment (FACT)

We assume responsibility for directly providing acute, active, and ongoing community-based psychiatric treatment with a multidisciplinary team approach. Within this program, the FACT team helps individuals maximize their recovery by developing goals, assisting clients through assertive outreach, rehabilitation, and support to gain hope and a sense of empowerment.

FACT provides psycho-social services primarily to adults with severe and persistent mental illness who often have co-occurring problems such as substance abuse or are homeless or are involved with the judicial system.

Florida Assertive Community Treatment (FACT) is referral based.
