Community Support Services

Community Support Services of Northside Behavioral Health Center provides a positive alternative to long-term hospitalization with residential, social, employment (vocational rehabilitation), and case management services.

Staffed by a multidisciplinary team of professionals, the Community Support Services program is designed to enable individuals to function as independently and productively as possible. Northside Community Support Services are available to adults 18 and over who are diagnosed with long-term mental illness and meet the other criteria for these programs.

Case Management Services

Staff members provide professional assessment, planning and monitoring of each client’s needs and continued advocacy.  The Case Management continuum consists of the following components:

Medication Services

Clients receive services related to medication evaluation and maintenance.

Centralized Case Management

Clients receive a variety of services related to linkage to appropriate community resources, advocacy for services, monitoring and assessment of needs.  Clients are generally seen one to two times per month.

Residential Services

A continuum of residential services from two group homes (most restrictive, 15 and 12 beds), supervised apartments (less restrictive, 40 beds), satellite apartments (least restrictive, 30 beds), and community apartments (no restrictions, 14 beds).  Rent payments are subsidized by HUD based on income.

Group Homes

This is where basic living skills are taught.  The ability to trust oneself and build upon individual strengths is encouraged.  Positive behaviors are learned and reinforced.  Group homes are staffed 24 hours per day.

Supervised Apartments

This is where living skills are practiced and enhanced.  Individuals assume greater responsibility for planning and maintaining living skills, become interdependent with others.  Supervised apartments are staffed 24 hours per day.

Satellite & Community Apartments

These facilities are where individuals live independently with minimal support staff.  Emphasis is on peer support and individuals helping each other solve day-to-day problems.

Supported Employment

Designed to provide job development and job placement for clients who are interested in employment.  Clients define what they want in a job and the Supported Employment staff helps them to match their needs with competitive employment positions in the community.

Florida Assertive
Community Treatment (FACT)

A multidisciplinary team approach that assumes responsibility for directly providing acute, active, and ongoing community-based psychiatric treatment.  Assertive outreach, rehabilitation, and support are also part of the approach.  The program team provides assistance to individuals to maximize their recovery, ensure consumer-directed goal setting, assist the persons served to gain hope and a sense of empowerment, and provide assistance in helping the persons served become respected members of their community.

The program provides psycho-social services directed, primarily, to adults with severe and persistent mental illness who often have co-occurring problems such as substance abuse or are homeless or involved with the judicial system.
